Frailty Crisis Hub
What is this service?
The Frailty Crisis Hub works together as One Team with GPs specialising in frailty, social workers, social prescribers, Community Response Team, Therapists, Age UK, and Urgent Responders. We provide care and a co-ordinated response to citizens in York that are vulnerable, frail presenting in times of crisis. This service aims to find an integrated solution to prevent unnecessary admissions.
