Bridging Discharge Support (YICT)
What is this service?
The team support other services in the community by bridging discharge support. We aim to expedite discharges using an evidence-based discharge assessment model. We liaise with services the hospital and support patients with crisis care and therapy needs.

- Monday to Friday 08:00-21:00
