Organisational Purpose

We’re here to make health and care work better together. Our role is to help join up services between GP practices, hospitals, and community care so that patients experience a smooth, connected journey without unnecessary barriers. We aim to grow as a trusted, collaborative provider that delivers high-quality, sustainable care across a wider area, while meeting national standards for integrated care. We also work to bring new ideas into community-based services, supporting GP practices to evolve and meet recognised standards. Everything we do is guided by fairness, accountability, and long-term sustainability. By focusing on the health needs of our communities and tackling health inequalities, we strive to make sure everyone has equal access to care and the best possible outcomes and experiences.
